Health Questionnaires
We ask you to complete and sign a health questionnaire form when you commence your classes with us. This requires you to disclose medical information. The Chartered Society of Physiotherapy makes this a legal requirement.
This is to ensure that you can partake in classes safely, but also to allow the exercises to be tailored to your requirements. It allows us to contact you in the event of a change to classes. Should you want to, you are able to withdraw consent at any time. This can be done in writing or verbally.
Attendance Registers
We document your attendance at our classes. We note any issues that you have reported to us either on joining the class, or indeed those reported on a class by class basis. We also note any modification we make to exercises for you. This is a legal requirement from the Chartered Society of Physiotherapy.
Satisfaction Questionnaires
Periodically we undertake a satisfaction survey. You are able to remain anonymous if you decide to complete the survey. This information is used for us to evaluate our services, and may be used on the website as a testimonial.
Securing Information
We are committed to keeping your personal information secure. We have put in place physical, electronic and operational procedures intended to safeguard and secure the information we collect, to prevent unauthorised access or unlawful processing of your personal data, and to safeguard against data being lost, destroyed or damaged.
Please note we cannot ensure the security of any information when it is transmitted to ourselves from other web pages. At your request we can send personal information to you via email.
Sharing Information
Information about you will be held securely and will be used only for the purpose for which it was collected.
Your confidential information may be required to be shared in accordance with UK law and guidelines from the Chartered Society of Physiotherapy. We will never share this data with third parties.
Retention of Personal Information
We will only keep your personal information for as long as it is necessary and in accordance with UK law and professional body regulations. The Chartered Society of Physiotherapy states that we are required to hold your personal information for 8 years after your last session with us. Children’s data must be kept until their 25th birthday. After this time your data will be destroyed securely.
Your Rights
You have the right to access your information and to ask us to correct any mistakes, and to delete and restrict the use of your information (please note that for legal reasons, we are obliged to retain your record for 8 years after your last class). You also have the right to object to us using your information, to ask us to transfer the information you have provided, and to withdraw permission you have given us to use your information.
